Insomnia and Incidence of Heart Failure in a Sample of Half a Million People: a Meta-analytic Review and Meta-regression of Prospective Studies

Purpose of Review Insomnia is perhaps the most under-recognized of the ten identified modifiable risk factors of heart failure. The current meta-analytic review aimed to explore the longitudinal associations between four types of insomnia symptoms including difficulty initiating sleep, difficulty maintaining sleep, early morning awaking, and feeling of non-restorative sleep and incidence of heart failure. Additionally, we explored the meta-regression of the time-dependent effects of follow-up duration on the incidence of heart failure. Thus, Scopus and PubMed were systematically searched for all articles published in English between January 1990 and December 2022 (PROSPERO-CRD42022378655). Recent Findings The systematic search identified ten studies containing 584,109 subjects. The pooled effect sizes obtained from six mostly high-quality studies (523,043 persons) showed significant longitudinal associations between all types of insomnia (except for difficulty maintaining sleep) and heart failure (HR ranging from 1.091 to 1.220). More symptoms of insomnia at baseline increased the risk of heart failure by 22%. The meta-regression results indicated that a follow-up time of 10 to 30 years is not a significant factor in the incidence of heart failure (P > 0.05). Summary The meta-analysis of several mostly high-quality studies addressed the significant longitudinal associations between different types of insomnia symptoms and the incidence of heart failure. The results of meta-regression on time-dependent effects indicate that long-term follow-up is not a significant moderator for the incidence of heart failure..
