Educational value from indications conferences at a hand surgery training program

Purpose To quantify the perceived educational benefit, examine attributable patient care changes, and describe the content of weekly surgical indications conference at a hand and upper extremity surgery training program. Methods Surveys were anonymously completed by three attending faculty, one hand fellow, and one senior resident (5 participants total) for 52 cases discussed over 6 months in 2019. The type of case, proposed treatment, diagnosis, and plan before versus after discussion were recorded with subjective data on educational benefit and helpfulness to clinical practice. Results The response rate was 100%. Most cases presented involved acute fractures (40%) or chronic fracture sequelae (e.g., malunion, nonunion) (21%). Attending surgeons presented cases for discussion primarily due to the complexity of the problem (39%), surgical approach or technique (35% and 31%, respectively), or controversy in literature about the topic (27%). The final care plan changed in 21 cases (40% total; major change in 19%, minor change 21%). Both the hand fellow and senior resident reported their approach to problem-solving/management changed in 71% and 74% of cases, respectively. The mean subjective helpfulness of conference discussions was 3.5/5. Cases that changed patient care were rated at 3.8/5, significantly different from cases that did not change at 3.4/5 (p = 0.03). Cases resulting in a major change scored 4.0/5 (p = 0.02). Conclusions Surgical indications conferences have a considerable subjective educational benefit to trainees. Most cases presented related to acute fractures or chronic fracture sequelae. Conferences also affect clinical decisions, causing adjustment strategies in 40% of cases..
