Cervicothoracic Presyrinx State Treated with Endoscopic Third Ventriculostomy: Case Report

Abstract The presyrinx state is a reversible spinal cord enlargement due to IV ventricle outlet obstruction. Its reversibility is linked to the restoration of cerebrospinal fluid pathways. Its presentation associated with hydrocephalus in the absence of Chiari malformation is infrequent. We present the case of an 11-years-old pediatric patient with a cervicothoracic presyrinx and hydrocephalus due to IV ventricle outlet obstruction without tonsillar herniation. We performed an endoscopic third ventriculostomy with complete resolution of the symptoms. Identifying a presyrinx state from syringomyelia is essential to define treatment. An endoscopic approach could be a successful therapeutic option for presyrinx and hydrocephalus due to IV ventricle outlet obstruction, even with cervicothoracic extension..
