Tyrosinkinaseinhibitor-assoziierte Choroidopathie
Zusammenfassung Es stellte sich bei uns ein 66 Jahre alter Patient mit seit 1 Woche bestehendem „Verschwommensehen“ beidseits vor. Aufgrund einer pulmonalen Melanommetastase erhielt er eine Kombinationstherapie mit Dabrafenib und Trametinib. Bei Erstvorstellung wurde rechts ein Visus von 1,2, links von 1,0 festgestellt. Es wurde ein normotoner Tensiowert beidseits gemessen. Fundoskopisch zeigten sich beidseits weiße, areoläre Veränderungen in der Aderhaut und den Aderhautfalten, links eine bullöse Aderhautamotio. In der OCT fanden sich beidseits neurosensorische Abhebungen sowie subretinale Flüssigkeit. Nach Reduktion der lokalen drucksenkenden Therapie und unter lokaler und systemischer antiinflammatorischer Therapie zeigte sich die Aderhautschwellung progredient, der Visus fiel ab rechts auf 0,5, links auf 0,1. Erst nach Absetzen der Therapie mit Trametinib kam es zu einer deutlichen Regredienz des Befundes. Der Visus stieg rechts auf 1,0, links auf 0,8. Die OCT zeigte rechts eine trockene Makula, links einen kleinen subretinalen Flüssigkeitsrest..
