Reduce the distance between knowledge and practice.

Ten years after the death of Alessandro Liberati - physician and researcher, founder of the Italian Cochrane Center - this article considers the changes that have taken place in international and Italian healthcare. In the first place, the authors regret the loss of cohesion of the group of researchers who have been working together for a long time in the Cochrane network, also due to the progressive change of the Collaboration, today more marketing-oriented than in the past. Evidence-based medicine is also suffering from the pressure of so-called "precision medicine" and from the bias that also affect randomized controlled trials: an important change in recent years is the greater importance given to the methodological quality of research, compared to theoretical value of the types of study. Alongside the bad news, there is also good news: the willingness to continue supporting the spread of evidence-based medicine among Italian professionals, which has given rise to several educational projects that discuss the opportunities and limits of innovation in medicine.
