严重寰枢椎后脱位三步综合救治：1例报告并文献复习 : = Three-step comprehensive treatment for severe posterior atlantoaxial dislocation: a case report and literature review
目的探讨严重寰枢椎后脱位的综合救治技术,总结采用该技术成功救治1例严重寰枢椎后脱位患者的治疗体会并进行文献复习。方法对合并强直性脊柱炎、齿突骨折的1例严重寰枢椎后脱位患者,按照其病理状态和疾病发展过程,进行呼吸衰竭救治、寰枢椎脱位救治和痉挛性瘫痪治疗等三步综合救治。结果经呼吸衰竭救治后病情初步稳定;颈椎牵引联合后路寰枢椎椎弓根内固定手术后寰枢椎复位良好,颈髓获得充分减压,四肢肌力和皮肤感觉功能恢复正常;行脊髓圆锥部选择性脊神经后根切断术(SPR)后肌痉挛消失,肌张力改善率100%,显著改善率65%。结论三步综合救治技术治疗严重寰枢椎脱位安全有效,但临床病例甚少,有待进一步积累总结。.
Objective To explore the comprehensive treatment technology of severe posterior atlantoaxial dislocation(PAAD), to summarize the experience of the treatment for one patient with severe PAAD, and to review the relative literatures. Methods For a severe PAAD patient with odontoid process fracture and ankylosing spondylitis(AS), three-step comprehensive methods including treatment for respiratory failure, PAAD and spastic paralysis, were performed according to the patient's pathological state and development process of the disease. Results Respiratory failure was controlled after the rescue. By cervical traction combined with posterior atlantoaxial pedicle fixation, satisfactory decompression of cervical cord was achieved, and muscle strength of limbs and skin sensory function recovered. Finally, myospasm resolved shortly after selective posterior rhizotomy(SPR) at conus medullaris with the improvement rate of muscle tension of 100%, and the significant improvement rate of 65%. Conclusion Three-step comprehensive treatment is a good and effective way to treat severe PAAD, while it still needs further cases accumulation and summarization due to very limited clinical cases..
