不同质子泵抑制剂对非静脉曲张性上消化道出血治疗效果的网状Meta分析 : = Network Meta-analysis of effects of different proton pump inhibitors on nonvariceal upper gastrointestinal bleeding
[目的]系统评价5种质子泵抑制剂在非静脉曲张性上消化道出血中的止血效果和安全性。[方法]计算机检索中国知网、中国生物医学文献数据库、万方、维普数据库、PubMed、EMbase、the Cochrane Library、Web of Science中有关质子泵抑制剂治疗非静脉曲张性上消化道出血的随机对照试验。检索时间均从建库至2019年1月31日。由2名研究者独立对文献进行筛选和资料提取并交叉核对,如遇分歧则咨询第三方协助解决。采用R软件的GeMET程序包和Stata 13.0软件进行数据处理。[结果]最终纳入文献146篇,共14 417例病人。网状Meta分析结果和排序结果显示:埃索美拉唑的止血有效率优于奥美拉唑、兰索拉唑和泮托拉唑,奥美拉唑的止血有效率低于兰索拉唑和泮托拉唑。埃索美拉唑的不良反应发生率低于奥美拉唑,奥美拉唑的不良反应发生率高于兰索拉唑和泮托拉唑。排序结果显示:不良反应发生率从高到低可能为奥美拉唑>兰索拉唑>泮托拉唑>雷贝拉唑>埃索美拉唑。[结论]网状Meta分析结果和排序结果表明:埃索美拉唑在非静脉曲张性上消化道出血中的止血效果优于奥美拉唑、兰索拉唑、泮托拉唑,安全性高于奥美拉唑、兰索拉唑、雷贝拉唑及泮托拉唑。.
Objective:To systematically evaluate hemostasis effects and safeties of 5 different proton pump inhibitors in the treatment of nonvariceal upper gastrointestinal bleeding.Methods:Randomized controlled trials about proton pump inhibitors in the treatment of nonvariceal upper gastrointestinal bleeding were retrieved from CNKI,CBM,WanFang Database,VIP Database,PubMed,EMbase,the Cochrane Library and Web of Science.The retrieval time was from the inception to 31 th January,2019.Literatures screening and data extraction were carried out independently by two researchers.A third party could coordinate differences in the cross-checking section.GeMET package of R software and Stata 13.0 software were used for data analysis.Results:Finally,a total of 146 references were included,involving 14,417 patients.Network Meta-analysis showed that the hemostasis efficiency of esomeprazole was better than that of omeprazole,lansoprazole and pantoprazole,the hemostasis efficiency of omeprazole was lower than that of lansoprazole and pantoprazole.The adverse reaction rate of esomeprazole was lower than that of omeprazole.And adverse reaction rate of omeprazole was higher than that of lansoprazole and pantoprazole.Sequencing results showed that incidence of adverse reaction from high to low might be omeprazole,lansoprazole,pantoprazole,rabeprazole,esomeprazole.Conclusions:The network Meta-analysis and sequencing showed that hemostatic effect of esomeprazole in the treatment of nonvariceal upper gastrointestinal bleeding was better than that of omeprazole,lansoprazole and pantoprazole.And its safety was also higher than that of omeprazole,lansoprazole,rabeprazole and pantoprazole..
