三种常见抗病毒方案对成人HIV/AIDS初治患者血脂的影响 : = Effects of three common antiretroviral regimens on blood lipids in treatment-naive adults with HIV/AIDS
目的:比较奈韦拉平(NVP)、依非韦伦(EFV)、洛匹那韦/利托那韦(LPV/r)联合两种核苷类反转录酶抑制剂(NRTIs)对初治HIV感染者血脂的影响。方法:采用双盲法随机选择初治的CD4~+T淋巴细胞计数<250个/μL的HIV感染者入组,通过Spw-Pb网络数据平台按1∶1∶1随机分配为A、B、C组,分别采用NVP、EFV、LPV/r为基础的3种抗病毒方案,对治疗前和随访过程中(12、24、36和48周)患者血清甘油三脂及胆固醇水平进行比较。结果:共入组300例,其中280例患者按照原方案完成随访至48周,3组患者基线信息匹配。在接受治疗48周后,C组受试者血中的甘油三酯水平分别比A组和B组高出1.07 mmol/L(P<0.01)和0.69 mmol/L (P<0.01);血清总胆固醇的水平亦高于其他两组,但差异无统计学意义。结论:在艾滋病初治患者中,LPV/r引起的血清甘油三酯升高较NVP、EFV更明显,临床医生需要高度警惕LPV/r相关高脂血症所带来的潜在风险。.
Objective:To compare the effects of Nevirapine(NVP), Efavirenz(EFV) and Lopinavir/Ritonavir(LPV/r) combined with two nucleoside reverse transcriptase inhibitors(NRTIs) on blood lipids in treatment-naive HIV/AIDS patients. Methods: Treatment-naive HIV patients with CD4+T lymphocyte counts<250 cells/μL were included in a double-blind randomization. All patients were randomly divided into groups A, B, and C by a ratio of 1∶1∶1 through the Spw-Pb network data platform. Three antiretroviral regimens based on NVP, EFV and LPV/r were used, respectively. The levels of serum triglyceride(TG) and total cholesterol(TCHOL) were compared at the baseline and during the period of follow-up(at 12, 24, 36 and 48 weeks). Results: A total of 300 patients were included, of which 280 patients were followed up to 48 weeks according to the original protocol. The baseline information of the three groups was matched. At 48 weeks after the treatment, the level of serum TG in group C was 1.07 mmol/L(P<0.001) and 0.69 mmol/L(P<0.001) higher than that in groups A and B, respectively. The level of serum TCHOL in group C was higher than that in groups A and B, although with no statistically significant difference. Conclusion: In HIV treatment-naive patients, the elevation of serum TG level caused by LPV/r is more obvious than that caused by NVP and EFV. Clinicians need to be highly alerted for the potential risks of LPV/r-related hyperlipidemia..
