长葛市2例寄宿制学校宋内志贺菌所致细菌性痢疾暴发流行病学特点及标本病原菌分离鉴定 : = Epidemiological Identification of Shigella Sonnei Dysentery Outbreak in Two Boarding Schools in Changge City

目的观察我市2起寄宿制学校宋内志贺菌所致细菌性痢疾暴发流行病学特点及标本病原菌分离鉴定。方法对我市2起寄宿制学校宋内志贺菌所致细菌性痢疾暴发事件进行调查,分析其流行病学特点,进行标本病原菌分离鉴定。结果 2017年10月至2019年6月我市2所寄宿制学校暴发宋内志贺菌所致细菌性痢疾疫情,共208例病例,以发热、呕吐、腹痛、腹泻等症状为主;小学生男、女学生病例性别比为1.28﹕1,初中生男、女学生病例性别比为1﹕1.21;2起疫情发生在夏季与秋季;腹泻病多重PCR检测系统对2起疫情的6份标本进行检测,结果均为Shigella/EIEC阳性;粪便标本宋内志贺菌检出率为64.86%。结论我市2起寄宿制学校宋内志贺菌所致细菌性痢疾暴发流行病学显示,疫情多发于夏秋季,小学生更易罹患宋内志贺菌所致细菌性痢疾,菌株分离鉴定粪便标本宋内志贺菌检出率偏低,而腹泻病多重PCR检测系统可准确检测病原菌,可作为临床诊断的重要措施,疫情暴发后需进行有效防控措施,以降低感染风险。.

Objective To observe the epidemiological characteristics of bacterial dysentery caused by Shigella sinensis in two boarding schools in our city and to isolate and identify the pathogens. Methods The outbreaks of bacterial dysentery caused by Shigella sonnei in two boarding schools in our city were investigated. The epidemiological characteristics were analyzed and the pathogens were isolated and identified. Results Two boarding schools in our city had outbreaks of bacterial dysentery caused by Shigella sinensis in 208 cases from October2017 to June 2019. The main symptoms were fever,vomiting,abdominal pain and diarrhea. The sex ratio of female students is 1.28:1,the sex ratio of male and female students in junior high school is 1:1.21;2 epidemics occur in summer and autumn;multiplex PCR detection system for diarrhea detects 6 specimens of 2 epidemics The results were all positive for Shigella/EIEC;the detection rate of Shigella spp. in fecal specimens was 64.86%.Conclusion The epidemiology of bacterial dysentery caused by Shigella sinensis in two boarding schools in our city shows that the epidemic is common in summer and autumn. Pupils are more likely to suffer from bacterial dysentery caused by Shigella sinensis. The strains are isolated and identified. The detection rate of Shigella is low,and the multiplex PCR detection system for diarrhea can accurately detect pathogenic bacteria,which can be used as an important measure for clinical diagnosis. After the outbreak,effective prevention and control measures are needed to reduce the risk of infection..
