2014-2018年重庆市中老年艾滋病抗病毒治疗患者生存状况分析 : = Analysis of the Survival Conditions and Associated Factors of Middle-aged and older HIV/AIDS Patients Receiving Antiretroviral Therapy in Chongqing City From 2014 to 2018

目的了解重庆市中老年艾滋病患者抗病毒治疗生存状况及其影响因素,为提高抗病毒治疗效果提供建议。方法在中国疾病预防控制信息系统艾滋病防治基本信息系统下载抗病毒治疗历史卡片,回顾性收集治疗2014-2018年重庆市中老年人群艾滋病病人基线和随访数据,以艾滋病相关疾病死亡为结局变量分析治疗生存状况。数据采用SPSS19.0软件进行寿命表法和Cox比例风险回归模型分析。结果共纳入治疗病人13 726例,平均年龄(62.5±8.4)岁,男性占70.7%,异性性接触传播占88.8%。因艾滋病相关疾病死亡1 379例,病死率为5.6/100人年。治疗第1年至第5年生存率分别为93.1%、89.1%、85.7%、82.9%和81.1%。多因素分析结果提示年龄、性别、婚姻状况、基线CD4值、感染途径、初始治疗方案、合并肺结核感染等因素与治疗病人死亡风险相关(P<0.05)。结论重庆市中老年患者艾滋病抗病毒治疗死亡风险相对较高,生存时间受多个因素影响,建议针对性强化中老年患者抗病毒治疗工作。.

Objective To understand the survival conditions and associated factors of middle-aged and older HIV/AIDS patients receiving antiretroviral therapy, and provide suggestions for improving the treatment effects. Methods Data of antiretroviral treatment of middle-aged and older HIV/AIDS patients were obtained from the AIDS prevention and control information system of China's disease prevention and control information system. Baseline and follow-up data were collected retrospectively. The survival conditions of treatment were analyzed with AIDS-related disease mortality as outcome variable. Statistical analysis(life table method, Cox's proportional hazards regression model) was performed by software SPSS 19.0. Results A total of 13 726 cases received antiretroviral therapy in Chongqing from 2014 to 2018. The mean agewas 62.5 ± 8.4 years old. 70.7% of cases were male, and 88.8% of patients were infected through heterosexual contact. There were 1 379 deaths due to AIDS-related diseases,with a mortality rate of5.6 per 100 person-years. The 1-, 2-, 3-, 4-and 5-year cumulative survival rates were 93.1%,89.1%, 85.7%, 82.9% and 81.1%, respectively. Multivariate cox proportional hazard model indicated that the survival time was correlated with age, sex, marital status, baseline CD4+T counts, route of HIV infection, initial treatment regimen and concomitant tuberculosis infection.Conclusion The mortality risk of elderly patients is relatively high during treatment, and the survival time was affected by many factors. This finding suggests that targeted measures should be conducted to improve the treatment effects of middle-aged and old HIV/AIDS patients receiving antiretroviral therapy..
