声门下联合口咽部冲洗对呼吸机相关性肺炎的影响 : = Effect of subglottic combined with oropharyngeal washings on ventilator-associated pneumonia

目的探讨声门下联合口咽部冲洗对呼吸机相关性肺炎(ventilator associated pneumonia,VAP)的影响。方法选择2016年7—12月在邢台医学高等专科学校第二附属医院重症监护病房(ICU)行机械通气治疗的44例病人为对照组,选择2017年1—6月44例行机械通气治疗的病人为观察组。对照组予以声门下冲洗,观察组实施声门下联合口咽部冲洗。比较两组细菌学检查结果、VAP发生情况、机械通气时间、ICU入住时间、人工气道留置时间、住院时间。结果观察组中速发性VAP、VAP总发生率(4.55%、22.73%)均低于对照组(18.18%、43.18%),差异有统计学意义(P<0.05);两组迟发性VAP发生率比较,差异无统计学意义(P>0.05);观察组中机械通气时间、ICU入住时间、人工气道留置时间、住院时间均短于对照组,差异有统计学意义(P<0.05);观察组中真菌、革兰阴性菌检出率与对照组比较,差异无统计学意义(P>0.0);观察组中革兰阳性菌检出率显著低于对照组,差异有统计学意义(P<0.05)。结论声门下联合口咽部冲洗能有效减少VAP发生、缩短病人治疗时间。.

Objective To explore the effects of subglottic combined with oropharyngeal washings on ventilator-associated pneumonia(VAP).Methods Forty-four patients who had received mechanical ventilation in the intensive care unit(ICU),The Second Affiliated Hospital of Xingtai Medical College from July 2016 to December 2016 were selected as the control group;44 cases who had received mechanical ventilation from January 2017 to June 2017 were selected as the observation group.Subglottic washing was applied in the control group,and subglottic combined with oropharyngeal washing in the observation group.The bacteriological examination results,incidence of VAP,mechanical ventilation time,length of ICU stay,artificial airway indwelling time and hospitalization time were compared between the two groups.Results The total incidence of medium-speed onset VAP(4.55%)and VAP(22.73%)in the observation group was lower than 18.18% and 43.18% in the control group,and difference was statistically significant(P<0.05);the incidence of late-onset VAP in the two groups showed no statistically significant difference between(P>0.05);mechanical ventilation time,length of ICU stay,artificial airway indwelling time and hospitalization time in the observation group were shorter than those in the control group,with statistically significant difference(P<0.05).The detection rates of fungus and G-bacterium showed no statistically significant difference between the two groups(P>0.05);the detection rate of G+bacterium in the observation group was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Subglottic combined with oropharyngeal washings can effectively reduce the incidence of VAP,and shorten the treatment time of patients..
