急诊PCI对RAAS系统的影响 : = Effect of emergency PCI on RAAS System

目的观察急诊介入治疗急性心肌梗死(AMI)患者早期血浆中肾素(Renin)、血管紧张素Ⅱ(ATⅡ)、醛固酮(ALD)的变化。方法纳入2017年至2018年入院治疗患者,其中急诊介入治疗AMI组(47例)、内科保守治疗AMI组(68例)、不稳定性心绞痛(UA)组(52例),对比分析各组血浆Renin、ATⅡ、ALD、脑钠肽(BNP)、射血分数(EF)、左室舒张末期前后径(LV)等指标的差异。结果急诊介入治疗AMI组Renin为16.83(7.77~42.84)pg/ml,内科保守治疗AMI组Renin为14.80(8.30~23.45)pg/ml,均明显高于UA组[7.97(4.22~13.85)pg/ml],差异有统计学意义(P<0.01),而对于心肌梗死组各阶段组间血浆Renin含量差异无统计学意义(P>0.05),各组间的ATⅡ及ALD对比无显著统计学差异(P>0.05)。Renin与ATⅡ具有相关性Rs=0.157(P<0.05),ALD与ATⅡ具有相关性Rs=0.325(P<0.01)。结论 AMI早期Renin水平升高,介入治疗对心肌梗死Renin水平影响无差异,ATⅡ、ALD无明显升高,但Renin、ATⅡ、ALD之间有相关性。.

Objective To observe the changes of renin,angiotensin Ⅱ(ATⅡ) and aldosterone(ALD) in the early stage of acute myocardial infarction(AMI) in patients with acute myocardial infarction(AMI).Methods 2017~2018 patients admitted to hospital were divided into AMI group(47 cases),AMI group(68 cases) and unstable angina(UA group)(52 cases).The plasma Renin(BNP),left ventricular end diastolic diameter(LV),ejection fraction(EF) and so on.Results Renin was 16.83(7.77~42.84) in the AMI group and Renin was 14.80(8.30~23.45) in the AMI group were significantly higher than those in the UA group(4.22~13.85).There was significant difference(P<0.01).There was no significant difference in the Renin levels between the MI group and the MI group(P> 0.05).There was no statistically significant difference between the two groups(P> 0.05).Renin and ATⅡ were correlated with Rs=0.157(P<0.05).AD was correlated with ATⅡ(Rs=0.325,P<0.01).Conclusion There was no significant difference in the Renin level between AMI group and intervention group.No significant difference was found in ATⅡ and ALD,but Renin,ATⅡ and ALD were correlated..
