剖宫产子宫瘢痕妊娠的超声诊断技巧及临床价值探讨 : = Ultrasonic diagnosis technique and clinical value of cesarean section uterine scar pregnancy
目的:探讨剖宫产后子宫瘢痕妊娠的超声诊断技巧以及对后续治疗方案制定的指导价值。方法:回顾性分析笔者所在医院妇产科于2016年1月至2018年12月间收治的剖宫产后被临床诊断为子宫瘢痕妊娠孕妇32例,最早孕龄5.0周,最晚孕龄9.0周;所有孕妇均接受经腹和经阴道彩色多普勒超声联合检查方式,从子宫内的妊娠物、妊娠位置、血流分布、宫腔宫颈情况以及子宫切口位置的肌层回声等方面归纳该检查的诊断技巧和对此类患者的临床价值。结果:在所有收治的32例瘢痕妊娠的病例中有20例停经后经超声检查首次发现而收住入院,占62.5%,最早为5.0周,最晚7.0周;还有8例临床高度怀疑疤痕妊娠,多次经腹和经阴道超声超声仅提示可疑疤痕妊娠;还有4例超声未提示异常发现,后因大出血收住入院确诊;后两者孕龄均在7.0周后。结论对于剖宫产术后子宫瘢痕妊娠的超声诊断最佳时机在孕5.0~6.5周,超声检查仍是诊断该病首选的手段,尤其是采取经腹部和经阴道多普勒彩色超声结合检查手段,对于病情在评估还有诊断方面能够更具优越性,同时在制定治疗方案还有后续调整中也有指导价值。.
Objective To explore the ultrasonic diagnostic technique of uterine scar pregnancy after cesarean section and its guiding value to follow-up treatment.Methods The clinical diagnosis of 32 pregnant women with uterine scar after cesarean section in our hospital from January 2016 to December 2018 was retrospectively analyzed. The earliest pregnant age was 5.0 weeks and the last pregnant age was 9.0 weeks. All pregnant women were examined by transabdominal and transvaginal color Doppler ultrasound, and the pregnant material, position of pregnancy and blood flow distribution were obtained from the uterus. The diagnostic techniques and clinical value of this examination were summarized in the aspects of uterine cervix and myometrium echo of uterine incision position.Results Among all 32 cases of scar pregnancy, 20(62.5%) were admitted to hospital after menopause, the earliest was 5.0 weeks, and the latest was 7.0 weeks. There were 8 cases with highly suspected scar pregnancy, only suspected scar pregnancy with multiple transabdominal and transvaginal ultrasound, 4 cases with no abnormal findings and confirmed by massive hemorrhage after admission, and the gestational age of the latter two cases was 7.0 weeks later. Conclusion The best time for ultrasonic diagnosis of uterine scar pregnancy after cesarean section is 5.0 to 6.5 weeks of gestation. Ultrasonic examination is still the first choice for the diagnosis of this disease. The combination of transabdominal and transvaginal Doppler color ultrasound can more accurately diagnose and evaluate the disease and has great guiding value for the treatment and prognosis..
