8例剖宫产术后子宫瘢痕妊娠超声误、漏诊原因分析 : = Analysis of the causes of missed diagnosis of uterine scar pregnancy after 8 cases of cesarean section
目的:通过分析我院8例超声误、漏诊的子宫瘢痕妊娠病例,寻找误、漏诊的原因,提高超声对该病的诊断率,避免误、漏诊的发生。方法:选择我院2016年3月-2018年12月期间,经手术和病理证实为瘢痕妊娠的31例病例中,对8例超声误、漏诊结果进行原因分析。结果:8例误、漏诊病例中,2例误诊为难免流产,4例误诊为宫内早早孕,2例漏诊团块型瘢痕妊娠。其中7例配合经阴道超声检查,纠正错误诊断6例,1例仍然误诊为宫内早早孕。结论:剖宫产术后子宫瘢痕妊娠,经腹部超声检查容易出现误、漏诊情况,经阴道超声检查,诊断准确率较高,但也有误、漏诊情况发生,未能明确宫内妊娠诊断时,应密切随诊,定期复查,必要时可结合核磁共振检查以协助诊断。.
Objective:To analyze the cases of uterine scar pregnancy caused by ultrasound errors and missed diagnosis in our hospital,to find out the causes of errors and missed diagnosis, to improve the diagnosis rate of ultrasound for the disease,and to avoid the occurrence of errors and missed diagnosis.Methods:From the 31 cases of scar pregnancy diagnosed by surgery and pathology between March 2016 and December 2018,the causes of 8 cases of ultrasound errors and missed diagnosis were analyzed.Results:Of the 8 cases of missed and missed cases,2 cases were misdiagnosed as inevitable abortion,4 cases were misdiagnosed as intrauterine early pregnancy,and 2 cases were missed for mass-type scar pregnancy.Among them, 7 cases were combined with transvaginal ultrasonography to correct 6 cases of misdiagnosis,and 1 case was still misdiagnosed as intrauterine early pregnancy.Conclusion:Uterine scar pregnancy after cesarean section is easy to be mistaken and missed by abdominal ultrasound examination.Transvaginal ultrasonography has a high diagnostic accuracy,but there are also errors and missed diagnosis.If the diagnosis of intrauterine pregnancy is not clear,it should be Close follow-up, regular review,if necessary,combined with MRI to assist in diagnosis..
