湖北省2016-2017年新报告的男性HIV/AIDS病例的流行病学特征分析 : = Epidemiological characteristics of male HIV/AIDS cases newly reported in Hubei Province during 2016 and 2017

目的了解湖北省男性艾滋病病毒感染者和艾滋病患者(HIV/AIDS)的流行特征,为艾滋病防治策略的调整提供依据。方法从中国艾滋病综合防治信息系统中筛选湖北省2016-2017年新报告的男性HIV/AIDS病例,收集所有病例的社会人口学特征、当年度首次CD4+T淋巴细胞(CD4)计数等资料,采用描述性流行病学方法分析男性HIV/AIDS病例的性传播方式和晚发现情况。结果 2016-2017年湖北省新报告6 290例HIV/AIDS病例,其中男性5 141例,占81.73%;感染途径以同性传播(50.09%)和异性传播(49.00%)为主;性传播方式在年龄、文化程度、婚姻状况、职业和户籍地方面的比较差异均具有统计学意义(P<0.05),低年龄和高学历同性传播比例较高;不同地区男性HIV/AIDS性传播方式的构成比较,差异有显著统计学意义(P<0.01),武汉和宜昌同性传播比例较高(67.92%和63.74%),恩施、十堰和黄石异性传播比例较高(74.54%、71.70%和70.69%);随年龄增加或文化程度降低,同性传播和异性传播的晚发现率均升高(P<0.05),>60岁组分别为50.00%和46.34%,文盲组分别为50.00%和49.48%。结论男性受到同性和异性性传播的双重影响,控制男性经性传播艾滋病成为综合防治中的重要挑战。.

Objective To understand the epidemic characteristics of male patients with HIV infection and AIDS(HIV/AIDS) in Hubei Province, and to provide evidence for the adjustment of AIDS prevention and control strategies. Methods New reported male HIV/AIDS cases in Hubei Province during 2016 and 2017 were selected from the China HIV/AIDS Comprehensive Prevention Information System, and data collected from all cases including social demographic characteristics and the first CD4+T lymphocyte(CD4) counts in the current year. Descriptive epidemiological method was used to analyze the sexual transmission and late detection in male HIV/AIDS cases. Results During 2016 and 2017, 6 290 HIV/AIDS cases were newly reported in Hubei Province, of which 5 141 cases were male, accounting for 81.73%. The infection route was dominated by homosexual transmission(50.09%) and heterosexual transmission(49.00%). Sexual transmission method had statistically significant differences in age, education level, marital status, occupation and household registration(P<0.05). The proportion of homosexual transmission was higher in low-age and high-education group. The composition of male HIV/AIDS sexual transmission in different regions was statistically significant(P<0.01). The proportions of homosexual transmission in Wuhan and Yichang were higher(67.92% and 63.74%,respectively), and the proportions of heterosexual transmission in Enshi, Shiyan and Huangshi were higher(74.54%,71.70% and 70.69%, respectively). The late detection rates of homosexual and heterosexual transmission all increased with the increase of age or the decrease of education level(P<0.05), 50.00% and 46.34% in the >60 years old group, respectively, and 50.00% and 49.48% in the illiterate group, respectively. Conclusion Males are influenced by both homosexual and heterosexual transmission. Controlling male sexually transmitted AIDS has become an important challenge in comprehensive prevention..
