中年冠状动脉三支病变患者的临床特点和危险因素分析 : = Clinical Features and risk factors of Middle-aged patients with Three-vessel Coronary artery Disease
目的探讨中年冠状动脉三支病变患者的临床特点和危险因素。方法入选中年不稳定心绞痛或急性心肌梗死患者68例(49.9±6.3岁),男58例,女10例。根据冠脉造影结果,将≥3支病变者作为试验组(21例),≤2支病变者作为对照组(47例)。回顾性分析其临床资料,采用二元logistic回归分析中年冠状动脉三支病变的危险因素。结果试验组低密度脂蛋白(LDL-C)和肌酸激酶(CK)显著高于对照组,差异有统计学意义(P<0.05),而BMI、血红蛋白及左室射血分数显著低于对照组;差异有统计学意义(P<0.05)。试验组他汀类药物、钙通道阻滞剂、高血压及糖尿病的发生率明显高于对照组;差异有统计学意义(P<0.05)。二元logistic回归分析显示,糖尿病、高敏C反应蛋白(hs-CRP)、高密度脂蛋白(HDL-C)纳入回归模型;差异有统计学意义(P<0.05)。结论糖尿病和hs-CRP是中年冠状动脉三支病变的潜在危险因素,而HDL-C可能是中年冠状动脉三支病变的保护性因素。.
Objective To investigate risk factors and clinical characteristics in middle-aged patients with three-vessel coronary artery disease.Methods 68 middle-aged patients(49.9±6.3 years) diagnosed as unstable or acute myocardial infarction, including 73 males and 13 females, were divided into test group(n=21,one-or twovessel disease),control group(n=47,three-vessel disease) according to coronary artery angiography. Two-way logistic regression analysis was used to determine risk factors of three-vessel coronary artery disease in these patients.Results Compared to control group, test group had higher levels in low-density lipoprotein cholesterol and creatine kinase(P<0.05),and lower levels in body mass index,hemoglobin as well as left ventricular ejection fraction(P<0.05).In addition,test group had higher incidence in statins,clacium channel blockers, hypertension and diabetes mellitus.Two-way logistic regression analysis showed that diabetes mellitus(DM),high sensitivity C-reactive protein(Hs-CRP), high-density lipoprotein cholesterol(HDL-C) were associated with three-vessel coronary artery disease.Conclusion DM and Hs-CRP level are potential risk factors in middle-aged patients with three-vessel coronary artery disease, and HDL-C level may be protective factors in such patients..
