1例不明原因肺炎病例密切接触者的病原学诊断与分析 : = Laboratory diagnosis and analysis on one close contact with a pneumonia case of unknown etiology

目的对1例出现流感样症状的不明原因肺炎病例密切接触者进行病原学诊断,分析实验室诊断过程应关注的问题。方法采集该病例的咽拭子标本,应用荧光PCR方法检测H5N1病毒、甲型H1N1和季节性流感病毒,多重PCR方法检测12种常见的呼吸道病毒。同时接种Hep-2细胞进行病毒分离,采用PCR方法扩增病毒的基因片段,进行序列比对分析。并应用间接免疫荧光方法检测病人急性期血清IgM抗体。结果荧光PCR方法检测H5N1病毒、甲型H1N1和季节性流感病毒核酸阴性,多重PCR检测到标本为腺病毒核酸阳性。应用Hep-2细胞分离到病毒,hexon基因序列比对结果显示,该病毒与GenBank数据库中的11a型腺病毒hexon的核苷酸序列同源性99.9%。间接免疫荧光反应还检测到病人的腺病毒IgM抗体阳性。结论此不明原因肺炎病例密切接触者为11a型腺病毒感染。另外,除考虑SARS-CoV,H5N1外,不明原因肺炎病例的实验室诊断还应关注其他导致重症肺炎的呼吸道病原体。.

OBJECTIVE To explore the important implications for the laboratory diagnosis of one close contact of a pneumonia case with unknown etiology.METHODS The pharyngeal swab specimen was collected for the extraction of nucleic acid used as the template for real time PCR and multiplex PCR for diagnosis of H5N1,pandemic H1N1,seasonal influenza virus and other common respiratory viruses.Subsequently,the specimen was inoculated to Hep-2 cell for virus isolation,which was used as a template for partial hexon gene amplification,sequence and alignment.Furthermore,indirect immunofluorescence assay(IFA) was performed using the isolated virus as an antigen and the acute phase serum of this patient as the primary antibody.RESULTS The multiplex PCR method demonstrated that this case was infected by adenovirus.The virus strain was successfully isolated.The IFA test showed positive anti-adenovirus IgM antibody.Furthermore,the nucleotide sequence of partial hexon gene showed as high as 99.9% identity with those of other 11a adenovirus.CONCLUSION Based on the epidemiological and laboratory analysis,the etiology of this infection was adenovirus serotype 11a.Furthermore,other respiratory viruses causing severe pneumonia should also be paid attention,besides SARS-CoV,H5N1 virus..
